Actually, the name Ingrid （イングリッド） appeared in Famitsu's previews for RE4 back in the day, so I don't think that was made up.

Yeah there's no issue with Ingrid Hunnigan's name in BH4. What I'm referring to is that "Ingrid" isn't mentioned at all in the BHDC script, and the character is just called "Hanna". I'm pretty sure it's another case of the translators being lazy and prone to assumption.
