Just found a really annoying "glitch" during the puzzle in the cells area, if you kill more than one keeper of madness at once basically you're fucked and forced to restart the level from the beginning. :-/ That happened to me too when I restarted from the checkpoint after killing two at once, but I found the 4th piece in the water near the emblem pedestal.
